A 56-year-old woman with no significant past medical history presented to the emergency department with worsening shortness of breath over 8 months. She had no prior history of tobacco use. Physical examination was notable for bronchial breath sounds in both lungs with oxygen saturation of 93% on 3 liters of supplemental oxygen.
